To become a member you must :
- Purchase five (5) shares for a total cost of $50. These qualifying shares are paid only once. Should you wish to resign, a full reimbursement of the shares is made or you may donate it to the Coop.
- As a member, you will accept to pay an annual fee, if required. The annual fees are determined by the Board of Directors according to the needs of the cooperative
- In 2014, annual fees were $25. Since 2015, a favorable financial budget enabled the Coop to suspend the annual fees.

- Fill in the application form.
- You can download the pdf form on the internet site or pick-up an application form at the Coop.
- Mail or leave the completed application form and your cheque for $50 paid to the Coop Santé de la Basse-Lièvre at :
